Spooner , Contracts Administrator C )sta DATE: July 23, 1987 CO^ SUBJECT: Approval of Standard Agreement #29-316-4 (State #87-91080) with the State Department of Health Services for the High Risk Infant Follow Up Project SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I . RECOMMENDED ACTION : Approve and authorize the Chairman to execute on behalf of the County , Standard Agreement #29-316-4 (State #87-91080) with the State Department of Health Services in the amount of $175 , 000 for the period July 1 , 1987 - June 30 , 1988 for continuation of the High Risk Infant Follow Up Project . II . FINANCIAL IMPACT : Approval of this agreement by the State will result in $175 , 000 of State funding (Federal Maternal and Child Health Block Grant #13. 994 ) for the High Risk Infant Follow Up Project . Sources of funding are as follows : $175 , 000 State (Same as FY 1986-87 State Contribution) 20,000 County Fixed Overhead in the Public Health Budget $195 , 000 Total Program III . REASONS FOR RECOMMENDATIONS/BACKGROUND: On April 28, 1987 , the Board approved submission of Funding Application #29-316-3 to the State Department of Health Services for continuation of the High Risk Infant Follow Up Project . Standard Agreement #29-316-4 continues this project during FY 1987-88 . The purpose of the project is to coordinate with Children' s Hospital Medical Center of Northern California , Regional Center of the East Bay, and other programs to serve high risk infants and their families . The objectives of the project are : ( 1) to identify infants 0 to 3 years of age who are at risk for development of mental , physical , or emotional handicapping conditions ; (2 ) to monitor/track their progress ; ( 3) to provide or facilitate provision of intervention; and ( 4) to evaluate the effectiveness of this intervention in pre- venting or ameliorating the effects of handicapping conditions . This contract has been approved as to legal form by County Counsel ' s Office . The Board Chairman should sign nine copies of the contract , eight of which should then be returned to the Contracts and Grants Unit for submission to the State Department of Health Services .
