SUBJECT: Approval of Standard Agreement #29-250-18 with the State
Department of Health Services (State #87-91603) for the
Dental Disease Prevention Program
SPECIFIC REQUEST(S) OR RECOMMENDATIONS) & BACKGROUND AND JUSTIFICATION
I . RECOMMENDED ACTION :
Approve and authorize the Chair to execute on behalf of the
County, Standard Agreement #29-250-18 with the State Department
of Health Services in the amount of $112 , 500 for the period
July 1 , 1987 - June 30, 1988 for continuation of the Dental
Disease Prevention Program during FY 1987-88 .
II . FINANCIAL IMPACT:
Approval of this agreement by the State will result in $112 , 500
of State funding (no change from FY 86-87 ) for the Dental
Disease Prevention Program. Sources of funding are as follows :
$112 , 500 State Funding
59 , 396 County Share
25 , 500 Private Donations
$197 , 396 Total Program.
III . REASONS FOR RECOMMENDATIONS/BACKGROUND:
On May 5 , 1987 , the Board authorized submission of a funding
application to State Department of Health Services for con-
tinuation of the Dental Disease Prevention Program. The
attached State-prepared contract will continue State funding for
this long-standing program during FY 1987-88 . This State man-
dated program is operated pursuant to Sections 360-373 of the
Health and Safety Code .
The goal of the Dental Disease Prevention Program is to reduce
the incidence of dental disease in children in selected school
districts and provide a comprehensive community-supported and
school-based dental disease prevention program. The program
serves 25 , 000 children in 90 public and private schools in
Contra Costa County.
This document has been approved by the Department ' s Contracts
and Grants Administrator in accordance with the guidelines
approved by the Board ' s Order of December 1 , 1981 (Guidelines
for contract preparation and processing , Health Services
Department) .
The Board Chair should sign eight copies of the contract , seven
of which should then be returned to the Contracts and Grants
Unit for submission to State Department of Health Services .
