As requested, please find the classification questionnaire for MHTS-C Level.,
.This supportive doct.unent. takes into account serious programatic decisions
reflecting the responsibilities and duties of the.-,C_-Level personnel in
h1HScreening as if affects the night. shift when the MHTS is the sole clinician
on duty as well as in lead capacity.
As you may be aware, within the past three (3) years there was a program
decision made to close the eight (3) bed Crisis Unit on E Ward, and move
the Crisis Services to Concord. Approx.i.mat.ely 750 of the MHTS staff assigned
to the hospital were transferred to Regional Clinics, however, resulting in
serious st--ffing problems on E Ward.

The :-TITS A-Level working under the direction of the MHTS-C Level was relocated
from the E.R. area to E Ward to begin what became a fully operative 24-hour
Crisis Service. The MHTS-A Level was replaced by two (2) psychiatric
technicians, and.we opened a five (5) bed unit in MHScreening. This change
put full lead responsibility for incoming patients; patients on the Crisis
Unit; consultation work to other hospital units and outside agencies on the
MHTS- C Level as the sole clinician on duty.
In addition, we began operating with 56 hours of 'P.I. (on-call) coverage to
compensate for loss of staff during the transition, and have maintained our
current staffing pattern for approximately 2z years.
The classification questionnaire more clearly describes the role and
expectation for the C-Level functioning in MHScreening, and eliminates the
need for 40 of the 56 hours of P.I. ocverage.

If public contacts are required either in person or by phone, state with whom,
and general,�or�iw a�purposes:
Majority of job consists of contacts with public, in person or by phone, either in
direct clinical intervention with patients and family; in consultation with other
agencies. Phone number is widely listed as crisis, consultation and referral line..
Calls from suicidal/homocidal, depresse //agitated and/or psychotic clients are
responded to, as well as calls/direct catact. with friends/families for advise,
referrals or intervention are responded to.
4iat decisions must be made in performing the work:
Assessment and treatment of mental bmd hclients. Decisions involve course and
level of treatment required in any given case. Continually making decisions
regarding diagnosis, treatmeht and disposition of clients. In any given session
with individual or family must make instant to instant decisions, based on
past experience and current observation, regarding therapeutic process, or
relationship itself. Knowledge and perceptions must be continually integrated
as basis for sound decisions which, as in cases with potentially suicidal or
homocidal persons, often involve life-and-death matter and matters relative to
the civil rights of both patients and their families.
Under W&I Code 5150, responsibility is given for determining patients who may be
a danger to themselves or others, or who may be gravely disabled and have been
brought to t=ithew Memorial by police or other agencies for that evaluation.
Evaluations and treatment is also required for the numerous clients who come to
Screening voluntarily but vhcsebehavior requires a subsequent 5150.
Decisions are made regarding clients who are intoxicated on alcohol or other
dangerous drugs which pcse both physical and emotional complications. These
cases required appropriate diagnosis in that misdiagnoses may compromise the
client's subsequent treatment and recovery. .Advanced working knowledge of the
DSM III is recruired for differential diagnostic decisions to- determine whether
a client is appropriately served in psychiatric services or requires further
medical evaluation.
Independent decisions are required to be made regarding the appropriate level
of care. These .may range :from a highly secured, structured acute psychiatric
inpatient setting to release to streets to assume own life style. Determinations
are made based on the clients' psychopathology versus current functioning.
These assessments may involve a recommendation for medication, evaluaticn and
referral to either a staff M.D. or an on-call physician.
What parts of the job require the greatest skill, knowledge and involve the
greatest responsibility:
Responsibilities include triage of all patients presented (direct contact/phone)
and accurate assessment in event of life/death situations (suicidal, high AWOL risk
toxicity problems, restraint issues, etc.) ; and to assure quality care of all
patients presently in Crisis Unit beds and in Reception area by alaocating work/
patient assignments to other staff on duty. As sole licensed mental health professional
provides supportiw./consultation role to all hospital patient care areas,

as well as outside agencies. Greatest skill and knowledge include full working
knowledge of Welfare &,Institutions Code; indepth knowledge of DSM III and its
application based on patient profile and mental status; ability to made advanced
psychological assessments with full range of clinical expertise to make
proper therapeutic interventions ranging from crisis work, to effective psycho- . : .
therapy sessions with seriously disturbed clients to complex family ther-ou
techniques. Position requires indepth knowledge of policies and'procedures_..
for Mental Health Screening, including not only patient este policies-and
administrative for E Ward but those that involve ambulances, police, transferring

40% As sole clinician on duty, exercises independent clinical judgement in the
evaluation and care of mentally disturbed patients. In this capacity provides
individual and family crisis therapy with children, adolescents and adults
covering a broad spectrum of psychopathology within the DSM 11I categories.
Provides written reports for inclusion in medical records.
20% Evaluates 5150 status by conducting full psychosocial assessments, including
full range of psychodiagnostic techniques. Makes independent clinical
judgement regarding acuity level of individuals, and makes arrm-igements for
admission to acute psychiatric hospital settings, both at Merrithew Memorial
and other hospitals, as appropriate. Prepares written reports of clinical
findings for medical records and for transferring facility use.
10% In lead capacity, provides clinical and programatic supervision to nursing
personnel as sole clinican on duty in MHScteening. Delegates work assignments
and oversees workflow. May provide clinical and programatic supervision
to MHTS-B level and clerical staff as lead worker in MHScreening.
10% Provides psychiatric liaison consultation services to the Medical Emergency
Room and Medical/Surgical Services, and provides written consultation reports
in sole clinician role.
10%, Provides consultation and evaluation both in person and on phone for wide
vanity of mental health professionals, M.D. , police/probation officers,
residential care operators, group home counselors, nursing home staff
regarding individuals in their care as sole clinician on duty. Provides
