FROM: SUNNE WRIGHT MCPEAK� Costa DATE: August 18, 1986 County SUBJECT: Smoking and Tobacco Use SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION RECOMMENDATION$. In order to promote greater wellness and reduce medical care costs, adopt the following position regarding smoking and tobacco use : a. Endorse the American Medical Association and Contra Costa Health Coalition goal for a tobacco-free society by the year 2000 and support the following actions : - .ban advertising of tobacco products require stronger health warnings on tobacco products - require labels listing ingredients in cigarette tobacco and other tobacco products - prohibit smoking on any common carrier - designate smokeless tobacco as a health hazard - eliminate federal price!-supports for tobacco and increase taxes on tobacco products support local ordinances to prohibit sale of cigarettes and other tobacco products to minors; to eliminate the distribution of free cigarette samples on public property, and. to protect the rights of non-smokers. b. Communicate the above position to our legislative delegation requesting support. c. Request the Smoking Education Coalition to develop a community education campaign to achieve the above objectives. d. Direct the Contra Costa Health Plan and request other health insurance plans marketing in Contra Costa to cooperate in an effort to identify the costs for smokers vs. non-smokers. and. develop rates that more accurately reflect the true expenditure experience. Request PSA to voluntarily prohibit smoking on all flights to and from Contra Costa County. BACKGROUND The Contra Costa County Board of Supervisors has taken a leadership role in efforts to reduce smoking and tobacco use. The current positions by. the American Medical Association and National Academy of Sciences to, among other things, banall advertising of tobacco and prohibit smoking on airples deserve local support to promote passage. The Contra Costa Health Coalition has adopted the above position. Attached is a news release which summarizes the position. Also attached areletters which communicate the position to the appropriate parties. The subsidy of smokers . by non-smokers in most insurance plans is as much as $100 per year. Because of the overwhelming request for non-smoking seats, your personnel declared the entire plane "non-smoking, " which produced a spontaneous outburst of cheering from most of the passengers .. I was impressed with how strongly everyone felt. I know you and PSA are pioneer entrepreneurs. Perhaps you would alsoconsider pioneering voluntary prohibition of smoking on your planes . The CCHS was founded in 1982 for the purpose of containing rising health care costs and promoting affordable quality medical care. It has highlighted wellness. programs and smoking prevention as key strategies for controlling escalating medical costs . In 1984 the CCHC launched a''. drive to achieve smoke-free work places for local employees and in 1985 its members joined the Smoking Education Coalition in securing passage of ordinances regulating smoking in all the cities and the unincorporated areas of Contra Costa County. The Contra Costa Health Coalition has also targetted health insurance rates for smokers and non-smokers as a key element of its newest anti-tobacco campaign. The CCHC is asking all health insurance plans marketing in Contra Costa County to disclose the medical costs of smokers vs . non-smokers and to reflect in their benefit rate structure more accurately the differential costs between smokers and non-smokers . The police contract in Holden, Massachusetts apparently goes to the full extent in this regard ( see news article below) . Can we go further in our own county? What has been proposed at the bargaining table so far? What further steps can be taken? RECOMMENDED ACTION: Refer to the County Administrator, County Counsel and Sheriff the contract in Holden, Massachusetts, to review and report back on its applicability for Contra Costa County.
