SPECIFIC REOUEST(S) OR RECOMMENDATIONS) a BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chairman to execute on behalf of the County, Standard Contract 424-373 with Crestwood Hospitals, Inc. in the amount of $194,605 for the period August 20, 1986 - June 30, 1987 for provision of intensive day treatment for mentally disturbed persons in need of subacute skilled nursing facility care upon discharge from acute psychiatric wards. This document includes a six-month auto- matic extension from June 305 1987 through December 31, 1987 in the amount of $112,391. II. FINANCIAL IMPACT: This program will be. funded by an increase in the County's Short-Doyle allocation pursuant to a negotiated settlement with the State Department of Mental Health which. transfers to Contra Costa County State funds previously allocated to Marin County for the operation of a regional skilled nursing program at Canyon Manor (Mental Health Management, Inc.) . (See attached letter from Mike O'Connor, Director, State Department of Mental Health, dated July 25, 1986.) Effective. imme- diately, the State Department of Mental Health has increased Contra Costa County's base Short-Doyle Allocation by $426,377 to be annualized in the amount of $568,503 in subsequent fiscal years. This new _Crestwood Hospitals, Inc. contract is funded through 90% Short-Doyle funds and a required 10% County Match. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: This contract will provide 25 treatment beds for chronically mentally ill adults who would otherwise require extended and more costly care at Napa State Hospital or the County's psychiatric inpatient facility. The development of this program to replace the County's beds at Canyon Manor in Marin County is in conformance with programmatic negotiations between. the State Department of Mental Health and Health Services Department personnel which resulted in the previously referenced settle- ment . Contract 424-373 provides for an intensive day treatment service to augment the services normally available at a Crestwood Hospital- Corporation locked psychiatric Skilled Nursing Facility for 25 additional Contra Costa patients at a daily rate of $25.66. Implementation of this contract will enable the County to terminate the existing regional agreement with Canyon Manor. Prior to this settlement with the Department of Mental Health, Contra Costa County was guaranteed access to .34 beds at the Canyon Manor facility at a daily rate of $56.00, however, these funds were not available to the County for alternative or more cost-effective programs. Contract #24-373 Board Order Page 2 Approval of Contract 4424-373 with Crestwood Hospitals, Inc. at less than half the daily rate at Canyon Manor will enable the Health Services Department to expend.the remainder of these funds for the expansion of critically needed local mental health pro rams. This Standard Contract has been approved as to legal form by County Counsel's Office. IV. CONSEQUENCES OF NEGATIVE ACTION: If this contract is not approved, the Health Services Department will be faced with two possible alternatives: (1) the County's access to subacute psychiatric skilled nursing beds will be dramatically reduced resulting in an increase in Napa State Hospital utilization and/or releasing seriously disturbed patients to the streets; or (2) the County would need to negotiate a contract with the Canyon Manor facility at more than twice the daily rate of the Contract 424-373 with Crestwood Hospitals, Inc.
