Spooner , Contracts Administrator DATE: July 31, 1986 CAlagy SUBJECT: Approval of Standard Agreement #29-316-2 with the State Department of Health Services (State #86-88774) for the High Risk Infant Follow Up Project SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chairman to execute on behalf of the County, Standard Agreement #29-316-2 (State #86-88774) with the State Department of Health Services. in the amount of $175,000 for the period July 1, 1986 - June 30, 1987 for con- tinuation of the High Risk Infant Follow Up Project. II. FINANCIAL IMPACT: Approval of this agreement by the State will result in $175,000 of State funding (Federal Maternal and Child Health Block Grant #13.994) for the High Risk Infant Follow Up Project. Sources of funding are as follows: $175,000 State 203000 County Fixed Overhead in the Public Health Budget $195,000 Total Program The prior contract with the State for this project last fiscal year was for $175,000, the same amount expected for FY 1986-87. This is the second year of a three-year project. No County matching funds are required. The only cost to the County is the $20,000 of fixed overhead. III. REASONS FOR 'RECOMMENDATIONS/BACKGROUND: On December 17, 1985, the Board approved Contract #29-316-1 with the State Department of Health Services for the High Risk Infant Follow Up Project operated by the Public Health Division of the Health Services Department. Contract #29-316-2 continues this project during FY 1986-87. The purpose of the project is to coordinate with Children's Hospital Medical Center of Northern California, Regional Center of the East Bay, and other programs to serve high risk infants and their families . The objectives of the project are: (1) to identify infants 0 to 3 years of age who are at risk for development of mental, physical, or emotional han- dicapping conditions; (2) to monitor and track their progress; (3) to provide or facilitate provision of intervention; and (4) to evaluate the effectiveness of this intervention in preventing or ameliorating the effects of handicapping conditions. This contract has been approved as to legal form by County Counsel's Office. The Board Chairman should sign nine copies of the contract, eight of which should then be returned to the Contracts and Grants Unit for submission to the State Department of Health Services.
