Compromise and Release COUNTY OF CONTRA COSTA Defendants It is found that applicant has been advised of the nature of the potential claim of his dependents, and has waived for himself and for his dependents any claim for death benefits arising out of the Injuries described herein. The parties to the above;entitled action having; filed a Compromise and Release herein, on May 1, 1984, settling this case for $ 25,000-00 in addition to all sums which may have been paid previously, and requesting that it be approved; and this Board having considered the entire record, including said Compromise and Release; now finds that it should be approved. IT IS .ORDERED that said Compromise and Release be approved. Award is made in favor of: WILLIAM M. MILLS AGAINST: COUNTY OF CONTRA COSTA PAYABLE AS FOLLOWS: One lump sum to applicant, less permanent disability advances which are continuing pending approval of compromise and release, and less $3,000.00 to Craig Dykman as an -attorney's fee.
