SUBJECT: Approval of Contract Amendment Agreement 424-736-21 with
Sunrise House
The Board on' November 1, 1983, having approved Contract 424-736-19
with Sunrise House for alcoholism program services; and
The Board having considered the. recommendations of the Director, Health
Services Department, regarding approval of Contract Amendment Agreement 424-736-21
to rescind the Contract Termination Notice approved by the Board on. June 26, 1984
and reduce the contract payment limit as part of the overall budget reductions in
the FY 84-85 Health Services Department Budget, IT IS BY THE BOARD ORDERED that
said contract amendment agreement is hereby APPROVED and the Chairman is AUTHORIZED
to execute the contract amendment agreement as follows:

Effective Date of Amendment: July 31, 1984 (no change in original
contract term)
Six-Month Automatic
Extension Payment Limit Decrease: $2,500 (from $72,928 to a new
total payment limit of
