(Give full details; use extra paper if required) Claimant was riding his bike on Pinehurst road when he hit a pot hole causing him to fall and suffer serious injuries. 4. What particular act or omission on the part of county or district officers, servants, or employees caused the injury or damage? Pinehurst Road is controlled and maintained by the County of Contra Costa. At the time of claimant's accident, Pinehurst Road had become physically damaged, deteriorated and defective to a degree that it became a dangerous condition to reasonably foreseeable users when using Pinehurst Road with due care. At the time of claimant's accident, Pinehurst Road was a dangerous condition as defined by Government f Y Code §830(a). Contra Costa County had a duty to maintain Pinehurst Road in a safe condition. 5 What are the names of county or district officers, servants, or employees causing the damage or injury?
