on June 30, 1975 your Board referred to the Director, flu-man
Resources Agency, for review a-d report a request received from
&r. Jack Estenson, Chairman, Board of Directors, Teen dope
Foundation, Inc. , (a youth facility for twelve in Concord)
requesting approval for a furthsr increase in their monthly
rate from $900 to $926 for the 1975-1976 fiscal year.
Annually child care inatizticr_s must submit their proposed
rates to the Bay Area Placement Committee. B.A.P.C . consists
of representatives from approxiUately 20 counties within and
surrounding the Bar. Francisco Day Area. This Committee deter-
mines an appropriate payment rate for Each institution based
upon the size and quality of staff, services provided and phys-
ical facilities. B.A1�P.C. originally approved for the 1°75-1976
fiscal year a rate of $831 per month for Teen Hope Foundation,
Inc. The institutionlappealed tfie approved rate, and on subsequent
review B.A.P.C. increased the recommended rate from $631 per .month
to $900 per month. The $900 per month rate was approved in reso-
lution No. 75/523, approved by your Board on June 30, 1975. It
must be pointed out that the $900 rate represents a 22 percent
increase over the 1974-1975 rate of $736 per month.
Based on review of the facts and implications of this matter,
I concur with the Director, Human Pesources Agency, in his recom-
mendation that the rate adjustment being requested by Teen Hope
Foundation, Inc., beldenied. Yxcordingly, I do not propose to
list this matter on my agenda.

The referenced Board Order referred to me for review and report to the
Board a request from Teen Hope for an increase in their rate from $900
to $928.
. I strongly recommend that the Board of Supervisors deny Teen Hope's request.
There are several reasons for this recommendation:
1 If the Board begins to consider every request for a rate
increase after the thorough analysis given by the Bay Area
Placement Committee (BAPC), BAPC's work will be seen by
institutions as only an interim step which can routinely be
appealed to the Board for adjustment.
2. Teen Hope has already been granted a 22% increase in rate
from their 1974Ii75 rate of $736 to the 1975-76 rate of $900.
3. Teen Hope was originally granted only a 10.8% cost-of-living
increase by BAPC. An increase to $900 was granted upon appeal
of BAPC's decision to the BAPC Steering Committee.
4. Any additionallrate increase made by this Board of Supervisors
is in effect binding upon all other counties, both by State
regulation and�by practice, under which counties extend the
courtesy to the county in which a facility is located of paying
the rate set by that county's Board of Supervisors.
Attached is a report from Mr. Jornlin on this subject, which recommends that
Teen Hope's request be denied. I endorse Mr. Jornlin's recommendation and
request that you present our recommendation to the Board of Supervisors on
July--22.

The BAPC approved board rate for Teen Hope Foundation, Inc. , adopted
by Contra Costa Board of Supervisors was $736 for Fiscal 19.74-75. In
planning for 1975-76, Teen lHope Foundation, Inc. , asked that their
official rate be established at $977. In the initial review of this request,
BAPC approved $831 as the rate commencing July -1 , 1975. BAPC advised Teen
HopeFoundation, Inc., as follows:

The recommended rate of $831 represents an $81 or 10.8/ increase
over your current rate) This is consistent with our guideline
of 10.8% increase in the cost of living. We cannot make payments
on the principal of your mortgage. We do not .find any specific en-
richment of your program which would justify the 30.2% increase
requested. We also note that your program operated at 82% of
its capacity last yeah. Our rate is computed on the basis of
90% occupancy. We find no justification for other than a 10.8%
cost of living increase."
Teen Hope Foundation, Incl , next requested a budget review through an appeal
hearing before the BAPC administrators steering committee. Issues for resolu-
tion were: I
"(1) Teen Hope's need for higher salaries for all its staff and
(2) The necessity of l doubling the mortgage payments to include
interest payments on a second mortgage previously. deferred. We
are. removing from out request the payment on principal ; thus re-
ducing our rate request to $967."
Appeals were heard May 28 and the rate for Teen Hope Foundation, Inc. , was
increased from $831 to $900. The difference between the BAPC recommended
rate and the request forleither -$967 or $950 was based on:
1 . The costs of a half time cook was disallowed.
2. The secretary allowance was reduced from half time to
quarter time. . I
3. It is our belief that some flexibility exists in the use
of 'as needed' help..
4. We did allow the $1 ,692 sewer service charge.
5. We used a 90% occupancy rate."

Teen Hope Foundation Inc.. was founded in 1971 as a non-
EXECUTIVE DIRECTOR profit corporation to work with youth in the high school
Stephen SardellaM.A. age 'in the drug abuse area. In 1973 it became licensed
under the State -Department of Health as a care and treat-
ment center for youth with a capacity of. 12 residents.
It has operated under the Bay Area Placement Committee
budget and board rates with residents being assigned
from Probation Officers and Juvenile Justice Courts from
the 16 Bay Area Counties.
In 1973 on the original budget makeup for acceptance
fob B..A.P.C . Irates an unrealistic low rate was' developed.
by, not having the experience, knowledge, or counseling
guidance bo develope a rate per resident that would make
survival possible.,
The deficits accumulated since makes continued operation
questionable unless a board rate is established that will,
assure enough. income to meet the operational expense.
The budget prepared for 1975-76 has a coverage of .our
program complete enough that the quality of our program
can be maintained. The board rate per resident requested
on our amended budget appeal on 4/2.1/'75 was for an amount
of $967 per Iresident per month. Since in this meeting
with the review board of B.A.P.C. an acceptable rate could
not be agreed upon, we appeared at another review with the

TYPE OF CHILD CONSIDERED FOR PLACEMENT
Preference will be given to youngsters with San Francisco roots. Children
must be able to be served by public schools, be of potentially normal intelli-
gence, not be severely physically handicapped and must have potential for
living independently in the community as adults.
DESCRIPTION OF RESIDENCE
The Group Home is an old 1910 Victorian, in the Bayview district of San Francisco.
This large house has four bedrooms, a very large kitchen, large recreation room,
living room, and a front parlor which serves as an office and individual counsel-
ing room. It is located in a predominantly black residential neighborhood and
is very close to transportation, shopping, the library, and Job Skills Center,
where some of the girls receive vocational training.

DESCRIPTION OF THE PROGRAM:
The Home: This Group Home was founded specifically for''third world)'San
Francisco youngsters, to be located in their community, close to churches,
community centers, cultural groups. It is envisioned as providing a "home" .
for troubled youngsters, or youngsters from troubled families, that is
accessible to family both physically and culturally in order to provide treat-
ment in the youngster's own community rather than in an all-white suburban or
rural environment. Many of the staff are'third world"San Franciscans who have
worked at other treatment facilities and provide both trained staff and
excellent role models for the residents. A home-like atmosphere pervades,
with the family joining in the dinner meal and evening and weekend recreation, ,
household tasks and general group activities, whenever possible. l
The volunteer program includes Office, Art, Tapia, Police Community Relations,
Bayview, young"third world"college students and a few successful self-made
community merchants. Volunteers help the youngsters participate in communityy
projects, "Y" activities, provide tutoring, a "big sister" relationship and
provide some financial assistance in the form of merchandise, i.e. , furniture,
clothing discounts, etc.
Training and staff supervision is directed: jointly by Consuela Hoskins, the
Director, and Barbara Campbell, L.C.S.W. Clinical Director, at weekly staff
sessions. Weekly therapy group is led by Ms. Campbell. Individual therapy is
available with Ms. Campbell or other therapists in the community.
The School: Youngsters can attend any of San Francisco's public schools, the
excellent Mt. St. Joseph, Homewood Terrace, San Francisco. Boy's Home School,
Simpatico,is available if the county will pay tuition. This is an excell-
ent EH school, current rates are $350.00 per month, including lunch.
TREATMENTPROGRAM

(3) Casework, group work, and family therapy to young women and their
parents when appropriate
(4) Medical and psychiatric care
(5) Psychiatric consultation
(6) A therapeutic milieu in a neighborhood based group home
(7) Assistance in developing healthy social_relationships with peers and
adults through a variety of techniques and design of individual programs,
in addition to the group living experience
(8) Recreation and cultural enrichment through the use of community centers,
volunteers, child care staff, and through individual and group activities
designed by staff to meet the needs of the youngsters
(9) Cultural tolerance and minority'group identification and group pride
through a carefully designed program introducing aspects of cultures r:
represented by residents, staff, volunteers, and the surrounding

REFERRAL MATERIAL PREPLACEMENT POLICY AND PARENTAL VISITS
Initial screening may be done by telephone. If placement appears appropriate,
the referring worker is asked to submit:

1. Court Report, initial and current
2. Psychiatric and psychological testing, if available
3. School and/or shelter hall reports
A visit and interview with Ms. .Campbell and .Mrs. Hoskins is expected of each
prospective resident. The staff would like the prospective resident to visit
for several hours, meet staff and residents and join them for a meal before
deciding to come to live there.
When placement is agreed upon, the following records must be submitted:
1:. Medical and dental history
2. Two consent forms - renewed annually
3. Current Medi-Cal card
COMMENTS AND RECOMMENDATIONS
Though this facility is only three months old, Y.O.U.R.S. is serving a need by
some of our recent immigrants from larger, urban centers. This worker respect-
fully recommends acceptance for general use of the Y.O.U.R.S. group home.

paid to child care institutions during fiscal year 1974-1975.is 'AMENDED to include
Berry Creek Ranch School, Post Office Box 31, Berry Creek, CA 95916, at a monthly
rate of $707.00 and Charter Oak Psychiatric Hospital, 19757 East Covina Boulevard,
Covina, CA 91722, at a monthly rate of $819.00, effective April 29, 1975•

There is attached for your information a copy of a memo-
randum submitted by J. A. Davis, County Probation Officer, dated
January 6, 1975, proposing an increase in the rates for certain
child care institutions. You will note that the adjustments are
submitted based on compliance of the institutions with the Fair
Labor Standards Act.
At this point it appears that "the county may have no alter-
native with respect to approval of these rate increases. However,
your Board may wish to review the matter; the additional cost for
the remainder of the fiscal year is $5,502.

Institutions
The Bay Area Placement Committee has completed a review of staff positions and
salaries paid.by several child care institutions and recommends the following
monthly rate adjustments. The adjustments are necessary since the institutions
must comply with the Fair Labor Standards Act.
Social Service departmental staff have been contacted on the recommended rates
and agree that the adjustments should be made with an effective date of January 1 ,
1975.

Dear Mr. Davis:
Community of Concern, .Inc, has 'operated residential treat-
ment centers for adolescents in the Richmond area for the
past four years. During that time, we have encouraged
maximum consumer participation in our homes, and attempted
to reduce all overhead expenses to a minimum. Consequently,
we have been recieving only $ 250,00 a"month per placement
from all referring sources,..
We are now forced to request a substantial increase in
funds from all referring agencies, and will be requiring
4525,00 a month per placement effective August 1, 1974,
This increase is necessitated by the termination of a:
three-year, 314D Grant for physical plant expenses:;
increasa- from $;5150,00 to $ 350,00 per month for two staff
persons; hiring of an additional part-time staff person
at ; 100,00 per month; and inflationary increase in all
operating and program expenses.
This increase will primarily affect ,the placement of
Juvenile Court Wards in Community House, 3146 Groom Dr, ,
Richmond, and occasional Court placement of more sophisticated
adolescents in Unity House, Richmond,
community of Concern continues to maintain active and ,.
productive liaison with community, city, county and state
services, thus enabling us to provide residents with-a
varied and individualized array of choices from which to
develop their own goals and objectives,

Billings for medical costs of placement in maternity homes
shall be made under the California Medical Assistance Program,
rather than to the County.
The rates listed above are applicable to all placements of
children under the jurisdiction cif County departments and are to
be considered maximum amounts with lower or going rates below
the maximum to be paid wherever fe'a.sible.
Placement in any institution whose monthly rate exceeds
$1000 requires specific approval by the Board of Supervisors.
This resolution cancels and supersedes the previously
adopted Resolution Nos. 73/471 and 73/�,7�2, and said cancellation
becomes effective July 1, 1974.

At shed is a suggested format for the rates effective July 1 , 1974. One
listing covering all rates is requested for fiscal 1974/75, as most juveniles
will be placed under AFDC-BHI funding.
We have coordinated effort on this list with staff of the Social Service
Department.
All rates have been either reviewed by the Bay Area Placement Committee, or
are still under. appeal process. The later rates are indicated by an asterisk
and may require adjustments upward.
I am attaching a copy 'of the June 3, 1974, letter from Mr. Herstein, which
indicates, as a result of recent Federal and State Wage Hour laws, that the
established rates may not cover costs of these facilities. I will keep you
informed on this matter.
Please obtain approval of the attached rates at the Board's next scheduled ,
meeting.

Recommendations
Dear Jim:
At a neeting of the California Association of Residential Centers held
on May 29 in San Diego, our membership heard a report from our attorney
regarding the impact of the New Federal and State Wage Hour Laws upon
the financial status of. our agencies. It is now-clear that exemptions
which formerly applied to 24 hour care institutions are no lon-er in
force, and that this will substantially increase the costs of residential .
treatment for the various agencies. For example, the law provides that
-employees who work overtime shall be paid .for time and half after 40 hours,
whereas previously, time aad a half payments started after 48 hours. The
Federal Law went into effect on May 1.
Since these developments occurred after our Agencies submitted their
budgets to the Bay Area Placement Committee, and after their fees for
July 1, 1974 Juane 30, 1975 were set by'BAPC, it is clear that some
Agencies will be Unable to r-aintain their operations without requesting
an increase in their previously agreed-upon fees. I understand that our
agencies will do everything possible to make necessary adjustments in their .
operations so as to keep these requests. to a minimi".., but I just wanted you
and the various Boards of Supervisors to be aware about this contingency
so that budgetary planning could take -it into account:.

Gentlemen:
This letter 'concerns Debra Williams, a ward of this court,
who I ordered placed `at, the Engleside. Mental Health. Center,
contingent upon this board-, granting support :in-.the amount of
$1,800 monthly.
I am writing this 'because I 'am concerned about Debra' s
demonstrated strong _self-destructive .tendencies, with which we
have been. dealing, unsuccessfully, for some: time. Earlier , ,
placements and programs. have all proved frustratingly inadequate
to reach. her and we arenow at a point where a locked facility is
a must if predictions of . tragedy !in .the near future are not to
prove accurate.
She is not delinquent, in any customary sense, and is, in
many respects, a. most appealing and capable young* woman; with luck
and effort she can be a, success.ful adult .and an asset to society.
However, we have evidence enough t:o indicate that a "low-key"'
treatment program will be unsuccessful and ,her a.d justment at our
juvenile hallis. suc,h that it is , apparrent that merely providing
security (a lock-up ) while hoping for some kind of near' spontaneous
maturation 7411 simply cause her mental health to deteriorate further..
There >is, reas'on to believe, that the extra 'monthly- costs to
the taxpayers of the Engleside facility will be offset by the
shorter duration of treatment under their intense program.

Recommendations
Dear Jim:
At a meeting of the California Association of Residential Centers held
on May 29 in San Diego, our membership heard a report from our attorney.
regarding the impact of the New Federal and State Wage Hour Laws upon
the financial status of our agencies. It is now clear that exemptions
which formerly applied to 24 hour care institutions are no longer in
force, and that this will substantially increase the costs of residential .
treatment for the various agencies. For example,- the law provides that
employees .who work overtime shall be paid .for time and half after 40 hours,
whereas previously, time and a half payments started after 48 hours. The .
Federal Law went into effect on May 1.
Since these developments occurred after our Agencies submitted their
budgets to the Bay Area Placement Committee, and after their fees for
July 1, 1974 - .June. 30, 1975 were set by BAPC, it is clear that some
Agencies will be unable to maintain their operations without requesting
an increase in their previously agreed-upon fees. I understand that our
agencies will do everything possible .to make necessary adjustments in their
operations so as to keep these requests to a minimum, but I just wanted you
and the various Boards of Supervisors to be aware about this contingency
so that budgetary planning could take it into account.
